Unsubscribe

I wish to unsubscribe the above practice from the Dentisan Clean Water Club with immediate effect. I understand that if my subscription is cancelled in the first 10 days of any month I may receive an order for that month and will be liable for any monies owing for the products received. This cancellation will remove ALL data from the Clean Water Club site and if the practice wishes to re-enroll, I understand that the data will need to be re-input.
