CERTIFICATION CRITERIA
CWC Certificates are issued for each individual surgery (NOT the whole practice). To maintain surgery certification a surgery must receive consecutive ‘Pass’ results. To qualify, new pass reports must be issued between 60 days and 150 days after the previous pass report date. Certificates are valid for 3 months.
Select the surgery from the drop down menu below and click SET Surgery. (The appropriate surgery should be selected and SET for each new certificate)
